Evaluation

Program evaluation is a required component of the Royal College of Physicians and Surgeon's MOC Program. Your feedback is valuable for the continuous improvement of the Sally Letson Symposium and is essential in developing a program that is relevant to your learning needs. Please complete an evaluation for each session that you attended to claim your CPD credits.

The online evaluation is available at sls2019.ca/cpd. You can also complete evaluations onsite by visiting the evaluation stations in the Confederation Foyer of the Westin Hotel.

How?

  1. Login with the email address used during registration (if unknown, contact [email protected]).
  2. Enter your Royal College ID number to facilitate credit transfer to your MAINPORT holding area.
  3. Evaluate and claim credit hours for the sessions you attended.
  4. After evaluating all of the attended sessions, download, print or email a copy of your certificate of attendance.

The evaluation site will close on October 21, 2019. Evaluations completed by this date that include your Royal College ID number will be transferred to your MAINPORT holding area by October 29, 2019. Please login to MAINPORT after October 29, 2019 to claim your credits.

Your feedback enables us to better understand your learning needs and plan for future educational opportunities. We appreciate the time you take to complete the evaluation for each session you attend.
